OHEA is seeking community members to participate on the CHIP Advisory Committee to help shape and approve the Multnomah County Community Health Improvement Plan (CHIP). The CHIP is an action-oriented plan outlining priority community health issues and how these issues will be addressed, including strategies and measures, to ultimately improve the health of a community. The CHIP Advisory Committee (CAC) will serve as the primary advisory group for the CHIP, incorporating resources and providing feedback throughout development of the plan.

The CAC will meet on a monthly basis until July 2016. Community members selected for the CAC are eligible for a $500 stipend. Interested folks should apply by November 6, 2015. Applicants will be notified no later than November 13th, 2015 if selected to participate. The application form is available here: https://docs.google.com/forms/d/1VhACiIbLEfWE0Ha0YkRakZzGz8W1YiE_PevWoqeDAas/viewform

If you have additional questions about the CHIP or the CAC, please contact Marilou Carrera at marilou@oregonhealthequity.org. Thank you!
